How to Create a Questionnaire Builder for Vendor Review Workflow

The Admin role can create and manage questionnaire forms as part of the vendor onboarding process. These forms help ensure that all necessary information is collected and documented consistently. Here are the steps to create a questionnaire builder for Vendor Review.

  1. Log in to your Mekari Expense account. Then select the “Workflow” menu.

  2. Next, click the “Vendors” tab.

  3. Then click “Create workflow”.

  4. In the Questionnaire section, click “Select questionnaire”.

  5. Then click “Create questionnaire”.

  6. You will be directed to the following page. Below is a detailed explanation of each column.

No. Feature Name Description
1 Questionnaire Name & Description 

Displays the name and description of the questionnaire. Click the pencil icon to edit the questionnaire information. 

2 Section &  Description Displays the name and description of the section used to group questions by certain categories. 
3 Question box Used to enter the questions that will appear on the questionnaire form. 
4 Question Type  Used to determine the type of answer that can be given, such as Text and other available types. 
5 Description Used to add descriptions or additional instructions to the question. 
6 Toggle ‘required’ Enable this option to make the question mandatory for users to fill out. 
7 Duplicate question Used to duplicate a question that has been created so you don’t have to create a similar question from scratch. 
8 Delete question Used to delete a question from the section. 
9 Add question Used to add a new question to the section currently being created. 
10 Add section Used to add a new section to the questionnaire. 
  1. Here is an example of a questionnaire that has been created:

     

    You can reorder the questions by clicking on the ‘six dots’ icon below.

  2. Then click “Publish”.

     

    Select “Save as draft” if you want to save the questionnaire without publishing it. Questionnaires saved as drafts can still be edited before publishing.

  3. The list of questionnaires you have created will then be displayed on the Select questionnaire page. You can view, edit, or delete questionnaires using the available icons before selecting the questionnaire to be used in the workflow. 

  4. Then click on the questionnaire, and select “Select”.

  5. The selected questionnaire will be used in the vendor review workflow and must be filled out by the requester when submitting a vendor request.
